Student Transcript Request

Students who need transcripts must do so through the Guidance Office.

Students seeking transcripts must provide the following information:

1.  A written statement requesting the transcript which includes the following information:

• Student name and year of graduation
• Date of birth
• Name and address of the institution to whom it must be addressed

2. $1.00 processing fee

You may e-mail your letter of request to Mrs. Cathy Simmons at simmonscl@mail.santarosa.k12.fl.us  or in person during regular school hours.  Please follow up with the payment in the guidance office.
